This happens when your certificates are not created.
Go to:
1. Ae Preferences->CuteDCPTools Preferences
2. Click Signer Certificate Setup
3. Then click Create Cretificates

In your audio multichannel setup, was the audio tracks set to standard or mono?
The standard channels gives you -3dB lower than mono.

In version 1.6.5 the basic video controls are possible to enable.
You can also get the same result by changing the sequence settings (menu Sequence->Sequence Settings).
